APPOINTMENTS & FEES:

All services are by appointment only.

 

Initial Consultation - $600

Initial appointments are extensive evaluations 80-120 minutes in length.  We will review your personal, family, psychiatric and medical histories, identify treatment goals, and create a comprehensive treatment plan for your care

50 minute sessions - $300

Follow up sessions consisting of medication management and  comprehensive psychotherapy.

25 minute sessions - $200

Follow up sessions consisting of medication management and brief, focused psychotherapy.

PAYMENT:

Cash, check, and credit card are accepted.  Patients are required to have a credit card on file, which is billed the morning of your appointment.  An invoice/receipt with appropriate diagnostic and procedural codes will be provided for you to submit to your insurance company.

CANCELLATIONS:

Appointments may be cancelled or rescheduled 48hrs in advance (not including weekends or holidays).  Appointments missed or scheduled without 48hrs notice will be charged the rate for scheduled session time.  Of note, many insurance companies do not reimburse for missed or cancelled appointments.  The 48hr cancellation policy is waived during weather emergencies. 

 

INSURANCE:

Medicare: Dr. Solazzo participates in Medicare.  Patients are asked to provide a copy of their Medicare ID upon scheduling their first appointment

 

Private Insurance Carriers and Medicaid:  Dr. Solazzo does not participate with private insurance carriers or Medicaid.  Patients may file claims for reimbursement with their insurance companies for out-of-network service.  Claims must be submitted by patients directly to their insurance companies.
